About Us

At Oakvango health care, we provide specialist care and support for adults with a neuro-degenerative diagnosis
and mental health who present with Challenging behaviour and associated complex needs.

We support everyone in our care to maintain life and social skills to enable them live as independent, meaningful
and fulfilled lifestyle, that is Safe, with a Good Experience and Positive Outcomes where they are valued,
respected, and listened to.

Our care model is designed to promote and maintain the day- to- day physical and mental functionality and
independence of our client group in terms of engagement and participation in their;

day to day ADLs(personal care and grooming, food preparation, laundry, cleaning, gardening)

Community access such as shopping, access to the gym, swimming, cinema, going out to coffee shops,
community events such as sports to enhance and maintain their sense of community and belonging.

The People We support

We Provide care and support specifically for ADULTS OF WORKING AGE UP TO 75yrs with AN EARLY ONSET of a neuro-degenerative diagnosis including mental health
but are still physically able and functional in terms of ADLs but are deemed unsafe to live at home on their own.

Due to their age, those we support are often not suited to be cared for in the traditional elderly residential or nursing care settings as such settings are usually too restrictive and
unsuitable to meet their complex and challenging needs due to early onset of their diagnosed condition and may have experienced placement breakdowns from such services.

We care and support those diagnosed with an EARLY onset of;

  • Dementia (Vascular dementia, Lewy bodies dementia, Mixed dementia, Frontotemporal dementia)
  • Alzheimer’s disease
  • Huntington’s disease
  • Parkinson’s disease
  • Korsakoff syndrome
  • Acquired Brain Injury (ABI)
  • Mental Health Needs (Including those on Section 117 1after care, and Section 172 Leave)

Our Services

Our facilities are community-based small to medium 6–8-bedroom ensuite facilities giving our residents a sense of a family unit,
living in the local community with the re-assurance of 24hrs care and support to keep them safe in managing their behaviour
and associated complex needs due to their diagnosis

Aims and Objectives

We aim to provide a service that enables those we support to:

  • ​ Have choice and control as far as practically and safely possible of how they lead their day-to-day lives because despite their challenges, just like everyone of us, people with an early onset of a neuro-degenerative condition and Challenging Behaviour are entitled to choice over how their lives is structured to make it as full to their individual preferences as anyone else’s
  • Be valued and treated with respected as an individual to enhance self-esteem, status and recognition
  • Maintain and enhance existing skills and abilities but also have opportunities to learn and develop new skills that will enable them to be as independent as possible despite their behaviour challenges and complex needs
  • Develop and Maintain social, Family and community relationships to be part of and get involved in their community with a sense of connection because love and belonging is a fundamental need for all of us.

Our Ethos

Our founding principles are based on promoting and maintaining independence for working age
adults with an early onset of a neuro-degenerative diagnosis because being independent is a
key factor in achieving our self-actualisation.

It gives us freedom and choice. In turn choice gives us empowerment and control over our lives
and any decisions concerning our care and treatment.

Using a person-centred approach, our focus is on promoting an independent life for each of our
clients, enhancing and facilitating their existing skills and abilities and developing new skills to
ensure they become more independent.

We achieve this by consulting and working with them to plan a holistic and bespoke package of
care, with specific emphasis on choice and a personalised plan of care.

  1. Section 117: after care is a legal requirement for the NHS and Local authorities to provide support to be people who have been detained under the mental health act the aim of which is to help people avoid returning to hospital) ↩︎
  2. Section 17 leave is a period of time granted to an adult who is detained under the Mental Health Act away from the hospital which can be indefinite, for a specified period or on specific occasions with the guiding principle
    of; least restrictive options, maximizing independence, empowerment and involvement, Respect and Dignity
